Ardor Health Solutions is looking for a Physical Therapist to join our travel team in a HOME HEALTH setting, in SAN RAMON, CA! This is a full time travel contract position.
Requirements include, but are not limited to:
- Active CA. HOME HEALTH license
- 2+ years of Physical Therapist experience
- Ability to clear a drug screen/background check
- This position begins on 05/18/2023 and ends on 08/17/2023

Requirements for success in this: / job in a Setting:
- Assess patient’s daily living/work-related skills and develop retraining programs with measurable objectives
- Work with patient in accordance with physician orders to improve patient’s strength, range of motion and function for optimal living
- Coordinate and oversee Physical Therapy Assistants and Home Health Aides performance in following plan of patient care
- Provide monitoring and education to staff, family and caregivers to improve patients participation in their rehabilitation
- Maintaining patient documentation in accordance with facility policy and procedures
- Effective communication skills to advise, consult and instruct members of the multi-disciplinary team in specific occupational therapy modalities and interventions
- Must have a current driver’s license and automobile insurance
- Must have graduated from an accredited school
- Active state / Therapist license or the ability to obtain the required state license
